Target Audience: Pharmacists

Pharmacist's Objectives:

• In the setting of an acute gastrointestinal bleed:
• • Identify indications for temporary interruption of anticoagulant and antiplatelet drugs.
  • Identify indications for reversal of anticoagulant and antiplatelet drugs.
• In the setting of elective endoscopy:
• • Identify indications for periprocedural heparin bridging.
  • Identify indications for postprocedural resumption of anticoagulant and antiplatelet drugs.
